Provide the phone number you would like programmed onto the tag. When scanned, the tag will allow someone to CALL the emergency contact.
OPTION 2 — TEXT
Provide the phone number you would like programmed onto the tag. When scanned, the tag will allow someone to TEXT the emergency contact.
If choosing the TEXT option, you may also include a short customized message containing important information about your child or loved one.
PLEASE NOTE: NFC tags have limited storage capacity. Customized text messages should be kept short and concise and include only the most important information. If your message exceeds the tag’s storage capacity, we may contact you to request a shortened version.
2. Name or Initials for the Front of the Tag
Please provide the first name or initials you would like displayed on the front of the tag.
If you would like the front left blank, please write “LEAVE BLANK.”
